Upon first starting the game, Boxten and Poppy are offered as one of the two starter Toons. Whichever one the player doesn't select, along with other Toons, can be bought in Dandy's Store with Ichor. (Ex; selecting Poppy at the start means that Boxten must be purchased, and vice versa). Additionally, most characters have prerequisites that must be fulfilled before they can be purchased, such as reaching a certain amount of Research on a Twisted or performing a certain action a set number of times.

Every round will begin with a base of two Twisteds per Floor, this amount increasing depending on the number of floors reached and the number of players remaining, maxing out at 6.
If a Skill Check is failed and a Twisted is nearby, the nearby Twisted will run to it and check up on it. Twisted Astro, however, will run to the Machine and drain progress.
Twisteds are split into five rarities, each weighted differently depending on the floor number: Common, Uncommon, Rare, Main Character, and Lethal.
- Common Twisteds possess no special abilities and usually possess statistics reflecting the statistical build of their Toon counterpart.
- Uncommon Twisteds typically possess special abilities, but none pose a direct threat to the player, instead providing more of a subtle threat that changes how the player interacts with the floor, such as Twisted Brightney being able to increase the chances of a blackout, or Twisted Razzle & Dazzle affecting how you traverse the floor.
- Rare Twisteds possess special abilities that pose a direct threat to the player, such as Twisted Goob and Twisted Scraps' long ranged attack. Rare Twisteds such as Twisted Flutter have an abnormally high detection range, and/or speed that will affect the player drastically.
- Main Character Twisteds are unique threats that interact with the player in unusual ways, usually completely changing the floor's progression strategy. Oftentimes, if uncareful or unprepared, floors containing these Twisteds can lead to the demises of many players in the round due to their adverse abilities. Many Mains usually have secondary or tertiary abilities, such as Twisted Astro and Twisted Vee, who will also affect machine completion; if you fail a Skill Check, Twisted Astro will drain half of the failed machine's completion whilst Vee will block your screen and hone in on your location.
- There are two Lethal Twisteds as of now, that being Twisted Dandy and Twisted Dyle. Lethal Twisteds are only available by completing (or failing to complete) certain tasks in a round, and have the ability to kill the player in one attack regardless of current Health. To summon Twisted Dandy, for at least three appearances in a round, don't buy from Dandy's Shop. This will lead to Dandy being agitated with the Player. After at least 3 appearances of Dandy's Shop in a row with no buying, Twisted Dandy will appear, making his minimum floor to spawn be Floors 6+. In Twisted Dyle's case, he only spawns if you pick the " TIME'S UP" Card, spawning you in a special map along with Dyle. Going back to Lethal Twisteds, they are often compared to Mains in that they have similarly bizarre mechanics while also (in Dandy's case) being "Main Characters" of the Dandy's World TV Show.
Most Twisteds roam the halls of the various floors of the Gardenview Center, wandering aimlessly. Twisteds (with few exceptions) will pause for about 2s when reaching a dead end in their pathing before turning around. Upon spotting a player, they will begin a chase. Chases will end if the player is able to remain out of sight for long enough, usually by hiding behind obstructions (except Machines, which do not block Line of Sight), or if the Twisted successfully attacks the player by coming into contact with them. A successful attack will normally result in one worth of damage (the only exceptions being Twisted Dandy and Twisted Dyle), and the Twisted will pause for around 3 seconds (excluding most Mains), giving the player ample time to run away. Any other Twisteds chasing the Toon will still pursue the player, so you must make distance to completely be safe. Roaming Twisteds will be alerted by failed Skill Checks and, if nearby, will run towards the source of the noise (Twisted Vee, Twisted Astro, Twisted Dandy and Twisted Dyle are exceptions; if they are roaming, they will come no matter where they currently are on the floor). They will not exhibit this behaviour if they are in a chase when a Skill Check is failed.
All roaming Twisteds have their own stats relating to speed (either in a chase or while roaming), detection range (reliant on the player's Stealth stat), and attention span (how long they remain chasing you when the player is out of line of sight).
Some Twisteds are special, requiring activation to attack players. These kinds of Twisteds can be stationary or may roam around. There are currently only 5 Twisteds that exhibit this behaviour, 2 of which are stationary, 1 more stationary twisted that changes where his two stationary attack positions are, which is Twisted Blot's ability, 1 which roams around, and 1 that exhibits both behaviours, but only attacks while stationary. The best way to avoid getting attacked by these Twisteds is tracking where they are and what the traps look like. The exception to this rule is Twisted Glisten, which needs someone to stay nearby to not get activated (unless it is currently Panic Mode)
A random Twisted will appear on the Daily Twisted Board at 12am GMT and for 24 hours will have a higher chance to appear during the game. This Twisted cannot be Common or Lethal, but can be event-exclusive as long as their event is ongoing. This can be helpful for obtaining research on a specific Twisted, or be disadvantageous when it is dangerous and unwanted. Note that previous days do not affect what will be on the board in future days.
When the player initiates a chase or encounters a trap Twisted, it will provide them with five Tapes and increase their Research progress with that particular Twisted by 5% (10% if playing as Rodger). This can only be done once per floor, so if you get chased by a Twisted more than once on a floor, you will only get 5% research for one encounter. If the same Twisted is encountered on a different floor, you will receive 5 more Tapes and Research progress will be increased.
Holiday Twisteds are special Twisteds that only spawn during their respective Events. They have mechanics, functions and spawn rates corresponding to their rarities, but they all have one special spawn trait, which is at least one Holiday Twisted is guaranteed to spawn on their Event's Holiday Floor, or every Floor that is a multiple of 5, and the Main Holiday Twisted appearing every 20 Floors.

Twisted Amount Scaling Per Floor
As players progress through Floors, the number of Twisteds that appear gradually increases, starting at two and capping at six. The number of Twisteds that appear is determined by the floor and number of players in the elevator when the room is generated

Twisted Boxten
Twisted Boxten is a roaming Twisted that possesses no special abilities, the same as Twisted Poppy and Twisted Cosmo. Reflecting his average counterpart, his speed, detection range, and attention span are all average.
Just like other Common Twisteds, you should try to get away from his sight if he spots you. Try to preserve 40-70 stamina to outrun Twisted Boxten, especially on later floors where you get chased constantly; running out of stamina will make you vulnerable to him and may get you killed.

- Almost all of the Twisteds bare mechanics or behaviours relate back to their Toon counterparts in some way:
- Twisted Toodles has a low attention span based on her description, which most likely comes from her age.
- Twisted Boxten and Twisted Poppy are acknowledged by their descriptions to be some of the most common encounters with easy to avoid mechanics, with the real Boxten and Poppy being the two beginner characters the player can choose from upon first starting the game. Both have simple mechanics and are very common in public servers.
- Twisted Brightney gives herself away with her red light, much like how the actual Brightney's ability can expose the location of Twisteds during a Blackout and has a larger light radius. She can additionally have a chance to cause a Blackout herself, whereas the regular Brightney's ability aids in dealing with Blackouts.
- Twisted Rudie also has a large light radius, like how Rudie has a modified light radius.
- Twisted Finn's mechanic, giving the player a slow debuff after completing a machine is a reference to his Toon counterpart which gives the player a speed buff when a machine is completed.
- Twisted Teagan steals Tapes from the Toons, while her Toon counterpart needs to collect Tapes for self-healing.
- Twisted Goob can pull Toons towards himself, and vice versa with Twisted Scraps (minus the movement), just like their playable counterparts.
- Twisted Flutter's agility and speed reflect her Toon counterpart that is fast and has an ability to dash quickly.
- Twisted Flyte's lower speed but higher attention span could reflect his interactions with his little sister and overall role as a lookout, where he admits Flutter is a faster flier than him and likes to keep an eye on others.
- Twisted Gigi can take items from your inventory, while her Toon counterpart's active ability grants a random item in your inventory.
- Twisted Razzle & Dazzle alternate between two different states, similarly to the playable's Comedy/Tragedy passive ability.
- Twisted Rodger's mimicry of a Research Capsule replicates the Toon counterpart's Detective passive ability which grants players more research, which also applies to capsules.
- Twisted Shrimpo's slow speed and high persistence reflects Shrimpo's poor stats and playability. His demeanour, especially prominent in his "lost sight" animation, which has him stamping his feet on the ground in frustration when losing a chase. This references Shrimpo's angry attitude. His mouth also appears to be sealed shut with Ichor, reflecting how his Toon counterpart constantly yells about how much he hates everything and everyone.
- Twisted Connie's ability to hide inside machines reflect how her Toon counterpart's allows her to become invisible and untargetable. She is also incredibly slow whilst mobile, alike to Connie's 1 Movement Speed.
- Twisted Ginger's slow speed and long attention span reflects on how her Toon counterpart has very high Stealth and slow Movement Speed.
- Twisted Pebble's high speed reflects its Toon counterpart's speed, being the overall fastest in the game.
- Twisted Vee's pop-ups are similar to regular Vee's Mic Check ability (information-based ability that is triggered with a cooldown) and running to failed Skill Checks is similar to her Camera Hijack passive ability (gain information on machines under a certain condition). These pop-ups also reference her role as a gameshow host, in which one must keep the audience's attention (i.e. "Gameshow — JOIN IN!" & "LOOK AT ME!") as much as possible.
- Twisted Shelly's debuff reducing Extraction Speed reflects how her Toon counterpart's Inspiration ability increases Extraction Speed. In addition, similarly to how Shelly likes extinct dinosaurs, Twisted Shelly's body is shaped and has a posture much like that of a theropod dinosaur.
- Twisted Astro's soft, quiet footsteps make him difficult to notice immediately, which reflects how the playable character has the highest Stealth in the game. His ability to regress machines and apply the tired debuff (decreasing stamina regen) can also possibly be a counterpart to how the playable character can restore the stamina of Toons around him.
- Twisted Sprout's ability to damage Toons at range reflects his Toon counterpart's ability to heal at range. His impressive height exaggerates Sprout's and his traumatised expression likely refers to how much he cares about others (the actions he takes as a Twisted are the exact opposite of regular Sprout's morals—hurting instead of healing).
- Twisted Dandy's possession of a unique spawn condition is similar to how Dandy can only be used by developers. In addition, his attack's power might be linked to Dandy's Shop, as Dandy himself sells three items that have the potential to heal or save you while Twisted Dandy has the potential to deal three of damage in one blow.
- Twisted Blot is able to summon his arms to attack to toons. Meanwhile, Blot can summon Blot Jr. to protect the other toons and himself from damage.
- Twisted Dyle's body is shaped like a snake, which is likely a reference to how his Toon counterpart has a (potential) fear or disliking of snakes. His speed increasing also could be a reference to how trains quickly speed up, linking to Dyle's interest in trains.
